Weight is regulated by more than discipline, so a medical plan starts by finding out which factors are in play.

Your first visit is a consultation and a blood draw: a lipid panel, HbA1c, a comprehensive metabolic panel and thyroid-stimulating hormone (TSH). Your clinician reviews the results with you before anything is prescribed.

02

Two options, one conversation

Both of the medications we prescribe are once-weekly injections given at home, and both belong to a family of medicines that act on the gut hormones that signal fullness to the brain. The difference is how many of those pathways each one works on.

  • Semaglutide works on one of those pathways, GLP-1. It is the longer-established of the two.
  • Tirzepatide works on two pathways, GLP-1 and GIP. It is the newer of the two.

Which one fits depends on your health history, your labs and how you respond, so your nurse practitioner prescribes it after your consultation and explains why. You never choose the medication from a menu, you are never locked into one medication, and your clinician can change course.

03

What to expect

If medication is part of your plan, you learn how to give the weekly injection before you leave. Dosing starts low and is increased gradually, against how you feel and what your check-ins show. Mild nausea is common early on and usually settles as the dose is introduced slowly. Anti-nausea support is available if you need it.

Check-ins are more frequent at the start and spaced out as things settle, and your dose is adjusted at each one. A healthy diet and regular activity are part of every plan, with protein and strength training emphasized to protect muscle.

Your check-ins, dose management and medication are part of your personalized plan, and the full cost is given to you in writing before you start. When it is time to hold, reduce or stop medication, your clinician plans that step with you, so the habits built along the way stay in place.

Ask your own question →
Can I choose semaglutide or tirzepatide?

You can share a preference, but the prescription is a clinical decision based on your history, your labs and how you respond. Some patients start on one and move to the other.

Do I need labs first?

Yes. The baseline panel (lipids, HbA1c, a comprehensive metabolic panel and TSH) is drawn here at your first visit and reviewed with you before anything is prescribed. Recent outside labs are accepted if they are current, and your clinician will tell you whether anything needs repeating.

What are the risks, and who shouldn't do this?

The most common side effects are digestive: nausea, constipation, diarrhea and reduced appetite, usually early and while the dose is rising. Less common risks include gallbladder problems and pancreatitis. These medications are not prescribed during pregnancy or breastfeeding, or to anyone with a personal or family history of medullary thyroid cancer or MEN2. Your clinician screens your history and labs before prescribing and reviews side effects at every check-in.

How much does it cost?

Your monthly price depends on the medication and the dose you are on. Before you start, you receive your price and the dose it applies to in writing, along with what is included. Your consultation fee is credited to your personalized plan.

What happens when I stop?

Some regain is common after any weight loss, especially when medication stops abruptly. That is why stopping is planned, not sudden: your clinician reduces the dose in steps, and the nutrition and activity habits from your plan carry on after the medication ends.
